the cost of your appointments/assessments, we may need to share with them some personal information, for example name, Date of birth, policy number, diagnosis and treatment plan. The information shared differs from one insurer to the other. Piccadilly medical will keep your personal information safe and secure, and only staff involved in managing your case will have access to your patient records. However, for data storage purposes it may be handled by personnel who are not directly involved in your clinical care. Practitioners will not disclose your Personal Information unless compelled to, in order to meet legal obligations, regulations or valid governmental requests.
Piccadilly Medical will process personal data during the duration of any service you receive from us and will continue to store only the personal data needed for two years after the contract has expired to meet any legal obligations. After two years all personal data will be deleted, unless basic information needs to be retained by us to meet our future obligations to you, such as erasure details.
